There were a small number of them and they still died constantly from disease and famine. They reproduce with the assumption that most of their kids will die.
If the only way these people can be kept alive is with constant foreign aid then its a losing battle. Eventually they'll breed to a point where no one can afford to keep them alive any more and then they'll die by the hundreds of millions.
