They can do scale but trying to bring everyone in some massive AVENGERS ASSEMBLE moment doesn't always work. At the end of Heaven's Feel, Angra Mainyu was still active and it was still a massive deal but the fistfight between Shirou and Kotomine is one of the best scenes in the VN.
