I may just be out of touch, but doesn't this seem like mental illness?

This is Damar Hamlin's back. I would understand a small tattoo, but to cover your back in a tattoo purely about yourself seems wild. I can't comprehend a situation where someone would do this and not have a serious personality disorder.
