The CWC pilot in Brazil in Y2K had good attendances. The other editions, in the old format, would normally only fill the stadiums in the finals. Euros say attendance would be better if they hosted it, but I doubt that. Maybe for local clubs and the final, but in general they don't care. Not to mention a 32 teams tournament is bound to have a good few games that don't attract many people.

If they had sold the tickets at that price from the beginning they might have filled the stadiums pretty well. It's a bit ridiculous they didn't do airplane style pricing where the early buyers get cheaper tickets to find what a reasonable price would be.
I don't know if they overestimated burger interest, overseas fans' levels of disposable, or just decided to take the loss on tickets to begin with.

The only way for Inter to be out is to lose as Monterrey wins on +2 goal difference with Urawa.

I know you have Barcelona but I have to give you the terrible news that there's basically no chance of Barcelona missing out on the next CWC, even without any changes.
The only reason they were left out this time was that they were in full crisis mode during these 4 years and missed playing UCL 2 times in that period. They've stabilized now, so there's almost no chance of Atleti outranking them again (meaning Atleti would be the one missing out going forward).
The only way to keep Barça out would be to remove all European slots or maximum per country slots, and hope that that manages to leave them out.
On the other hand, English teams like Liverpool, Arsenal, Chelsea, ManCity etc. would all be competing for only 2 slots unless different clubs win the next 3 UCLs, so if any changes to qualification rules are made, it will be for them.

I meant "removing all extra slots", as in giving only 4 slots to European teams that win UCL (as is the case in most confederations); or alternatively reducing the 2-club per country cap to 1 club per country

Like I mentioned, Barcelona is going to be there just because it's bound to be one of the 12 best UEFA teams and 2 best Spanish teams in the near future. The problem for Infantino is finding out how to ensure that the popular teams elsewhere make it for sure (Liverpool, Arsenal, Tottenham, America, etc.). He definitely wants them in based on popularity but he'd like to keep it seeming fair rather than arbitrary like you mention.
