The Elmore art is obviously to set the tone of the game, it's near the start of the book.
The 5.5e orcs is for the species section of orcs of the PHB, all of the species sections illustrations show only characters of the same species looking like actual NPCs so it'd be retarded to expect them to look like a PC party.

I think it depends on the group. My highschool friends are more into the modern style. My friends from a gaming club I'm part of tend to be more into the older aesthetic.

I think this because the gaming club people are more interested in gaming and they want the world to be believable.
My highscool friends are more interested in the acting side of things and often want to be epic untouchable gods and don't really care about verisimilitude.

Personally I loathe to run the more modern style but when I can convince actor style players to get in on my grittier games I find that they can add a lot of great drama.
