As a DM, I tend to consider the stat block presented in the book as a standard representation of that sort of enemy. I will tweak it if I feel I want a slightly more powerful enemy of the same time as a sort of leader of the pack.
And I always increase the hitpoints of things if I have more than 4 players.
I do almost always re-write boss statblocks almost from scratch just because 5e is so fucking terrible at making them interesting.

Far as I'm concerned, a monster stat block is just a resource to be used and abused freely by the DM outside the session. Changing stuff on the fly mid-combat is a no-no, but if I'm prepping an encounter ahead of time I will change shit around or even entirely rewrite/refluff stat blocks to fit whatever I have in mind. Heck, I might combine several monsters into one if I want to make a multi-phase boss encounter.
