Is there something like writing a character monster book, but instead of being monsters the book is about designing around 256 npcs, with their own character profitle, name, concept art.

So, something like that can be used as the blueprint for another game or derivative media?

Yes, but they usually seem to be things made by players & GMs, and may be missing some of the elements you suggest.

What I want is to make a world with 256 female npcs, and without males, except the MC and his twin brother.

But make it so every npc has a fully flesh out story, psychological profile, concept art, game attacks and stats.

As far as I know modules usually only have couple dozen NPC that are directly related to content of the module, but you could compile NPCs across multiple modules to get the amount you need.
Or RNG it, either by hand or with a script.

That sounds like plenty of bookkeeping, couldn't you start with smaller prototype to see if it works and then scale it up? Say 4 families to begin with.

I'm using godot to code a generator of random ideas generated with chat gpt, so I can avoid writers blocks.

So it will take some months of work, but I think I can make it this way.

Chrysogon's Coterie for BFRPG might be what you're looking for, though idk if you have a particular system in mind or what. it's apparently a collection of almost 300 NPCs with classes, backgrounds, motivations, quirks, connections, etc. it's made for BFRPG so if you want to fluff them for a setting that isn't fairly generic old-school D&D fantasy it might take a little work, but the framework is there. full disclosure, i haven't used it myself, so it might be dogshit, no idea.

That's a start, with a bit of automation it shouldn't take months but perhaps just weeks.
I'd suggest looking into the basics of relational database use for keeping tracks of who-knows-who.
Using dere types as your lorem ipsum is perhaps a bit silly.

My idea was more a parody of harem tropes, because I wanted to make initially an rpg maker eroge.

But then I realized making it as a videogame would take too much effort.
So I decided to look up alternatives, and decided to make a PDF instead.
