Labyrinthus is unisex in that it doesn't assume player gender, but I wouldn't really expect you to get the kind of game you're looking for out of it.

Bithes for Thoth Amon is almost exactly what you described (it's under the last link in the OP), but I found it a bit lacking the last time I checked it out. There's been some updated but I haven't played it myself so I can't give any guarantees for quality.

Everything else is female/submissive-PC only ime.

There's always the option of making a system yourself but that requires you to invest the time and effort and also needs you to not be a retard when it comes to game design. I tried but failed at the effort part.

You can use a staple of other pbta games by picking an stat to finish the fight. Iron for damage, Edge for capture, Shadow for blackmail, etc.
If you want more involved breaking/combat, then port over Rhapsody of Blood style mechanics. You pick certain stats to make "openings" and then one to make "hits" (or just make "hits" outright if that's too slow). Combined with the previous flavor you can get something like:
You aim for the bandit's "Pirate Queen" trait that allows her to bring cronies as a complication. You attack it with Shadow, ridiculing her in front of everyone. It works, making some run away and others stand still because they hold grudges.
As a bonus, let's say you change the trait some way to reflect what happened.

I put a few pieces of flavor in it for various races, towns, deities, etcetera. Lifted some ideas from other people here that had discussed social and cultural issues. The second book can be a bit dense, but the first book is mostly tables and lists. I enjoyed reading it, but then again I enjoy reading RPGs in general, and I love 3.5e, so this is my brand of autism. YMMV. My advice is to just start with some combat simulations. One thing that GURPS does well in my opinion is that characters are really easy to create, so its at least easy to get started.

I've also never did anything too special with it, like vehicle combat etc. so I don't know how those things stack up. Also I just tend to use quick & dirty rules for shit like that. Aint nobody got time to be tracking how many masts my ship has left.

LAanon added quite a few tables to IK, plus I have always found it to be the most feature complete region, so I would reccommend giving it a whirl, and it's the natural starter point for both Saurian wilds (the new hotness) and Blood Moon - depends on if you like your lewd flavoured Pirates of the Carribean or Castlevania. I haven't myself yet played the former since it's more a high level run, currently bringing an Elf mage(picrel AI gomen) up to the task.

I would also again reccommend a beastgirl, LAanon added racial varieties to them. Classes haven 't changed that much in an of themselves, so if you want something new, a monk is a good bet.

And that is without the new homebrew. Reccommend definitely the Slime and Tentacle cave expansions.

Why does this branch of the hobby attract the most braindead simpletons? If I want to run a 'mainstream' RPG but play up the sex elements, that doesn't mean I want to ignore all the inconvenient parts! Those enhance the experience for me. I get that wouldn't be everyone's cup of tea, but then why are mainstream RPGs still so relatively popular?

Instead of using a simpler, freeform system that'd do a much better job for their needs, I end up dealing with players that whine I treat it as a normal game and GMs that balk at the idea. if both fail then the conversation stalls and it gets awkward
try again later under different circumstances
if one succeeds and both characters already seem interested then maybe you set up a date or something

serious failures lower interest by a couple steps
if this leaves a character uninterested then needless to say nothin is gonna happen
