Like also it just opens up a can of worms in the setting that probably these people didn't really think about thoroughly. For example, can't you force person to become an opposite gender for an arrange marriage (i.e. Only two daughters, so one of the family pays off a priest to forcefully sex-change.) The spell doesn't say anything about "willing" targets (but I guess consent is "gatekeeping" to these people).

This could become darker by the way, just because of the "permanency by siring." You could rape a person and forcefully make the spell permanent. I mean if that's the point of the design so be it and all to you, but it's still kinda of cringe fetish or it was not really well thought out.

Fully changing a person's physical sex to the point that they can bear/sire children is not quite the same as tranny shit, either way. No. Trannies feel a thing called gender dysphoria, simply put they feel that they are biologically the wrong gender, and they try everything they can to get as close to their desired gender through hormones and stuff.
