Making them ugly is the only based choice. Even better if they are based on some undesireable or lesser known animal. My next game is going to include the ancient race of flying buttock worms who are in war with the Ramisyllis multicaudata folk.

You might be able to run with one that's an outcast in some way. I think it was Book of Exalted Deeds that had an example of a good Mindflayer? Rare and extraordinary but possible.
