A few things, but mostly how quickly your tama's mood and food bars drain. On the Paradise, it can take hours for them to be completely depleted depending on their stage in life. There's been times where I've forgotten my Paradise at home before I go to work, and I come back and he's only at 1/6 on the food levels.
Overall it seems the device is made to be more of a "there when you want it" kind of thing, as opposed to demanding your attention or face Total Tama Death. Of course, tamas on the Paradise can still die. It just takes a long time and you have many chances to correct it before it actually does die.
