Hard Target with JCVD by John Woo was rated NC-17 and I honestly don't get it as it's just a generic "most dangerous game"-gun fu action with no sexual or suggestive material at all and very little gore (if anything, I mean they cut an ear from some fattie).

The american rating system is completely retarded.
Take all movies above PG13 and imagine them on a scale from 1 to 10.
1 is a fairly tame movie that's just a bit too much for PG13.
10 is the most depraved distressing shit you've ever seen (or it's basically just porn).

In the US, 1-9 gets an R. It's uselessly vague and tells you almost nothing about what to expect. 10 gets an NC17 and nobody is willing to touch it.
Most countries instead have one rating for 1-5 and another for 6-10. Often something like "15+" vs "18+". The higher rating doesn't have a stigma, everyone is happy to show and sell those movies.

Why hasn't it been changed yet? There's already an informal idea of "soft R" vs "hard R", why not just make that official by switching to R15 and R18 and drop NC17? Surely it'd be better for everyone?

The MPAA rating system is a grift. They don't tell you any actual hard rules, and you have to pay them each time you want to show them a new cut of your movie. Then you have to hope and pray that the review board composed of the wives of the studio heads are all having a good day or they might give you a hard R because they're having personal issues. Of course, the big studios get to sidestep all this by greasing the wheels, but if you're having to beg the review board for a lower rating, you're not part of that club.
