Think about it this way

For most of the show, we see Tony wrestling with the ideas of morality, wanting to be a “good person” while also being a gangster. Incompatible things because being a mob boss is inherently bad and will mean you do and sanction horrible things that hurt a ton of people

He tries to square this circle by acts of seeming kindness or mercy, like when he helps out artie, or doesn’t kill that pedo coach. But they don’t actually change anything in the long run

By the final season and his killing of Christopher, he accepts he is no longer trying to evolve or change and is happy to fuck over others for personal gain.

In this very episode we see Tony orders asbestos to be dumped in a lake instead of it being safely disposed of because he didn’t want to pay any more percentage to Phil. We also see AJ emotionally distraught over his little crew of friends stomping out the Somali student.

Tony in contrast murders his nephew, and his long run of bad luck gambling wise ends. He sees this as a sign given the peyote high. That he doesn’t have to worry about trying to do good and should be even worse

1. Existential Clarity / Acceptance of Chaos
Tony may be realizing that life is meaningless, chaotic, and random, and the only rational response is to embrace it, or at least stop resisting. This is consistent with his earlier existential musings (like "Is all this shit for nothing?"). The peyote opens him to the possibility that there is no grand meaning, and he's okay with that.

2. Freedom Through Amorality
He may have realized that killing Christopher was a good thing for him, and he's at peace with it. The guilt is gone. He "gets" that detachment and self-interest — not moral struggle — is the way to survive. In other words, "I get it: kill or be killed, and don’t feel bad about it."

3. A Spiritual Awakening (Twisted)
Some interpret it as a kind of spiritual moment, like a sick version of enlightenment. Tony feels transcendent joy because he thinks he’s finally above it all, beyond guilt, beyond consequences. It's his "Buddha under the Bodhi tree" moment, but with mob violence instead of meditation.

4. Pure Irony
It’s possible David Chase included it as ironic. Tony thinks he’s had a profound realization, but he hasn’t changed at all. The show then leads toward his downfall. So “I get it!” could be just another delusion: his version of enlightenment is just selfish, empty, and doomed.

The sun flashing mirrors and reminds him of the light house flashing in his coma dream, in which he gets to experience the life he would have had without the mafia and his background. He now gets that all his shit in life is because his of parents, trying to appease them, even after their death, but now is able to live freely. Like most psychedelic insights, though, the positive effects from this experience do not last long or form stable fundamental changes, and soon enough Tony reverts back into his old ways, regardless of remembering some of the insights he retains
