As soon as they walk out, they're asked if they want to take part in a poll and they provide their score. So you get an audience's immediate reaction to what they just saw.
Of course, there are caveats you have to take in mind. For starters, this is only polling for domestic audiences. So if you're trying to figure out what international audiences are thinking, that's going to be a bit harder of a task.
And while Cinemascore can be useful in trying to gauge word of mouth, it's also important to note that people have to see the movie to begin with. So you have a lot of instances where some movies have gotten high scores, but not enough people made the trip to theaters for that to make much of a difference.
