Letterman had a rep as an extremely miserable prick when he wasn't on air. He treated assistants and anyone he viewed as 'subordinate' like crap while sucking up to people he saw as superior in status (famous people he'd speak with an such).

At one point he was doing standup somewhere and there was a bit involving a life sized David Letterman blow up doll. He ended up having a breakdown on stage, beating up and tearing the doll apart and then storming out of the building never to return.

In the later era of his show you can see this very obviously, where the humor of it is seeing the level of disdain he has. Letterman begrudgingly get through his monotone introduction to the show, not even bothering to break for applause, face etched with frustration and gloom.
