The books actually have a perfect example in Thoros of Myr. He starts out a jolly fat man and then when next he's seen his skin is hanging loose around him. It's just that his rapid weight loss is also because he used part of his energy and life force to resurrect Beric just like Melly's shadow magic and fire sacrifices uses the life force unborn babies and people. Which is why Melly shouldn't have been able to resurrect Jon in the show: because she's actually an old woman under her glamour, she has no life force to give up like Beric did.

A character coming home and realising that the clothes they left behind no longer fit is always kino. I can't remember if it's in the movie but in Jan Guillou's Evil the first chapter focuses on the MC's little school gang and how much importance they put on these cool matching jackets they've all got, which they bought with the money from their criminal enterprise of stealing records and reselling them. When he comes home at the end of the book he finds the jacket in his room and tries it on and he can't even get his whole arm in before it starts splitting at the seams, showing that even though he was a terror at the school yard at the start he must have still been a little kid with plenty of growth ahead of him not just metaphorically but physically. Although I don't think he really grows as much as he thinks and that he'll continue to find reasons to use violence and terror for good, even though he keeps telling himself it'll be the last time.

Suspension of disbelief.

In the world of ASOFAI dragons exist. White walkers exist. Magic exists. But humans still mostly follow the laws of physics and other "laws" that humans IRL follow also. People can starve in GOT, there are skinny and fat people in GOT, so that means they get skinnier or fatter depending on how much food they eat and the exercise they get.

Sam being a big fatass despite everything he does breaks suspension of disbelief because, unlike other characters in the series, he seems immune to body changes from food intake and exercise. Dragons and white walkers maintain the suspension of disbelief because they are real in the context of the series. People are willing to pretend that these things exist since the series established them. The series never established that big fatasses will just always stay fat despite the food they eat or exercise the get.

I've got the opposite and so from my teens I've had basically zero hunger, I've only gotten some semblance of regular hunger feelings now that I'm in my thirties. I just kept eating despite feeling so full that I could puke because otherwise I'd die from malnutrition, and stayed away from certain foods because I couldn't afford to fill up on water, you know? So I never understood why the people who have the opposite and so feel hunger more often can't just do the opposite of what I did. I swear that game was filled with stuff that would make so much more sense if the apocalypse had just happened, but then they plonked the main quest story on top and made them fit together sloppily. Like how every place is filled with skeletons from when the blast happened, somehow still in the exact same position as when the bomb dropped after 400 years despite people living right next door or the place being open to the elements, as opposed to down in an untouched bunker.

Finding a ghoul boy in a fridge and then taking him back to his family's house where you're not sure what you're going to find could have been way more interesting.

As a fatass, I made lots of excuses over the years. 6 months ago I got heart problems and got a triple bypass (REY BYPASS!). Since then I've lost about 100 pounds. The trick? Having to be 5 hours without eating so I can take my sugar levels 3 times a day. Basically not eating all the fucking time. After a while (2-3 months) I stopped being hungry like I used to and it's not hard to eat normally.
Motivation. Not eating between meals. Eating a little less during meals. That's all. I still eat desserts, chips, etc, but much less. I eat more fruits and veggies, but not that much more.
My blood work is now perfect in every way.

I had what you had until about 28 years old. It was such a chore to eat food and working out at the gym requires a lot of food and it was absolutely impossible to eat enough to gain any muscle, hell I was lucky to not keep losing weight. I really had zero concept of how people could gain weight or even enjoy over eating, that is just so insane to me. Then I got hurt and now I drink and can eat such an absurd amount of food and still feel like I haven't eaten all day, I'm talking 2 frozen pizzas, a full box of Mac n cheese, and a pint of ice cream in one sitting and I could easily eat more. Still not fat though, fuck fatties.

Sam was never going to become a hardass and the whole point of his arc is it makes more sense for him to become a scholar/maester and he couldn't do that until his dad disowned him. The only time I've eaten ice cream for dessert every day was when I was put on this medication meant to help me sleep that I later found out they also give to anorexics because apparently the number one side effect is that you never feel full. It was really fucking strange to just be walking around and thinking "oh you know what? I think I'll have another slice of pizza, or a bowl of ice cream, or a sandwich to chew on while I watch a movie" when before every meal was a chore. I gained weight so fast that I still have stretch marks on the inside of my thighs. I was still never actually fat, though. I just got a bit overweight, felt my pants getting tight and one day I saw my chest in profile in the mirror when getting out of the shower and saw a hint of a moob forming. That was enough to give me this massive wave of dysphoria, like it was ridiculous how sick the idea of having tits made me feel, and I immediately cut back on snacks and desserts. It seems these guys get that same wave of dysphoria when seeing their body but instead of stopping it they drown those feelings in even more food? Shit makes no sense to me.

I can excuse someone being a bit overweight even if it's a survival situation. There's no need to have every guy in the Night's Watch lose weight during their forced march. But Sam wasn't just a bit fat, he was fucking obese. All those guys need to do is skip a few meals and the pounds melt off them, which is why they go on silly diets and think it's fantastic because just after a day or two the bathroom scales tell them they've already lost weight, but then when the weight stops disappearing that fast they grow disheartened and start eating again.

Even from the start it's pointed out that the only reason he's not losing weight at the wall to begin with is because he's quickly put in a servant role that doesn't have him do anything strenuous, so he's eating the same rations that everyone else does but they're all spending the days lugging ice blocks and doing sword exercises while he's looking after an old man and feeding some ravens.

fictional universes rely on internal consistency or else you end up with a world that feels non-deterministic, and thus, uninteresting.
