The scene where he's with all the mobsters wearing his own self-tailored clothes inexplicably slathered with grease paint works because every one of those hardened psychopath thugs realize the joker is completely batshit for really no reason and is an actual problem. I can't picture the scene with a more "grounded" "gritty" "realistic" joker having the same impact. In the interrogation scene, Joker's makeup getting scuffed parallels Batman losing his own grasp, it's not needed, but it's a nice touch. In fact, the opening scene, when Joker takes off his mask to reveal just another hidden identity, wouldn't be as memorable if he takes his mask off and reveals just some fucked up ugly freak.

I don't believe we missed an opportunity, I think the director made the right call

there's a really edgy joker comic where the character is supernatural and super sadistic and wears loose skin on his skinned face. It's what this reminds me of and I hate it.

They improved it.

As a few sober anons have pointed out earlier - all the make up (except the scar prosthetics of course) also was put on by Ledger himself - giving him that self made look plays really well to how the character is written in function to the script.
As his character basically is the one factor that continually sidelines all tropes and expected narrative devices - the fact that he looks completely self made grounds him from appearing as a deus ex machina.
