That's what I thought after seeing the film. But in the book it's clear she's a feminist who gets off on emasculating and humiliating men.
The Black orderlies are also violently abusive, and implied to be sexually abusive.
Chief Bromden points out that she specifically picked Black men who hate Whites to work on her floor, knowing they'd be more cruel to the patients.

The movie omits this, as well as the contrasting segment that shows another floor not under Ratched's control where things function fairly normally and the patients aren't being humiliated.
