the entire interaction is unpleasant but the waitress is the first to cross the line of what is acceptable in this situation. jack orders the chicken salad sandwich and asks her to hold everything, including the chicken, essentially willing to pay extra for two slices of toast while not asking her to break any rules (though his annoyance and contempt are clear in his tone and body language) and she responds, "you want me to hold the chicken huh?" snarkily, which is inappropriate. Jack escalates things with the "hold it between your knees" comment and is completely out of line sperging out at the end.
