My guess about the final prophecy is that it's something like "the girl must die to save the day", and Susie either voluntarily sacrifices herself OR, if Noelle is the girl, breaks the prophecy to save her by dying in her place. Either way, the ultimate result is that Susie dies.
Why do I think that? Because it gives the player an actual motivation to do the snowgrave/weird route.
The game is doing its very best to make the player love Susie. The weird route is basically about grooming Noelle into being the sacrificial lamb to save Susie.
Think about it - the Knight captures Undyne and is going to sacrifice her, and was seemingly going to capture Toriel before Undyne barged in.
What do Undyne and Toriel have in common? They're both powerful and they're both girls. Ralsei even specifically refers to Undyne as "strong" when you talk to him about her being held prisoner in the shelter.
What happens during the weird route? You make Noelle stronger.
Completing the weird route almost certainly won't save Susie - she'll probably die fighting you in the end instead - but the point is that a player who's become extremely attached to her might want to try.
If Noelle is actually the one who dies at the end and Toby thinks people will be motivated to do the weird route to try and save her then he's an idiot. Most of the fandom would happily throw Noelle into the woodchipper to save Susie and Kris, and use the weird route as just another opportunity to fuck her over.
