Unironically what similarities are there between SoP and TPP other than the main character twist?
I'd argue they are actually the most diametrically opposed games in the series.
MGS2 is a lightning quick and very dense game, you can't take more than two steps without running into a cutscene containing some mad revelation that turns the story on its head before you've even had time to process the previous one.
And this has a knock on effect on the gameplay as well where the game's large interconnected map goes mostly to waste since it's only used for a small mission at the start.
MGSV on the other hand is so slow paced that you'll probably have forgotten what you're even working towards whenever you unlock a new mission that actually progresses the story, you go hours and hours without anything significant happening.
Its two singular maps already start wearing out their welcome before you're even halfway through the game and the story never ramps up at all to make up for how tedious and repetitive the gameplay becomes.

PTSD is very real, but PTSD most often manifests through the trauma of loss combined with the thought of "it could have been me / I could have done something to save them." Survivor's Guilt, primarily. A dude who lost some good buddies in war is going to be fucked up. Serious injuries and lost body parts will also do it.

It is far less common to suffer PTSD just from something spooking you. near-death experiences, you doing grueling shit, or you getting to see enemies dying. Different kinds of lingering misfires in the brain happen there after you're no longer in a combat settings, but training would have gotten you ready to kill people who are complete strangers to you.
