While it looks like Marina is sacrificing Levi, If you break down the title screen you'll realize Marina is actually performing the same Vitruvia sacrifice that Daan tried to make for Elise, but successfully. Levi's throat is slit but only trickling blood instead of gushing. Theirs no enemy in the game that leaves you half blind, black smog blinds both your eyes. The title screen art is a call forward. The Old Gods die but not really because they represent "pure" ideas. Even if the gods themselves become disgusted and disinterested by humanity, what they stand for still exists in a bastardized sense within us so they can't really die and just exist in their diminished forms, acting as parodies of their true, purer selves. New gods are admittedly jobbers with limited power, and most of them just sit in the hall and do illuminati shit once their tenure is done. They lack the aformentioned purity to command and represent concepts that exist throughout the human canon like the Old Gods can. The whole god fusion/absoprtion thing is kind of weird but the Girl using the God of the Depths to become the God of Fear and Hunger is explainable; the Girl was purely gestated in the Dungeons so fear and hunger - amplified by the presence of the God of the Depths - allowed her to harness that purity of a concept unlike the New Gods. When you kill Depths in an ending A playthrough, the things it represents such as vermin, bugs and corrupting darkness still exist, but become an aspect of fear and hunger.
