They push/flank harder with molotovs etc so you have to constantly reposition, or react insanely fast. You have to make every shot count, headshots or knee shots for a stun. You can only take 2/3 shots before dying at full health. Rifles/shotguns end you even quicker.
Ammo and resources are (finally) survivor horror tier. Most fights if you shoot leave you with a couple bullets, again forcing stealth and waiting for the right time to takedown enemies.
Basically one, maybe two wrong moves you die and it's back to checkpoint. If it sounds miserable it is, I died 200-300 times on my grounded run. But you do learn something each time, like better dodge timing against infected, or how soldiers are deadly accurate if you move while they're peeking from cover.
A single encounter might take you a dozen tries to clear but when you finally crack the puzzle, kill each enemy in succussion without error, (or by the skin of your teeth) it's extremely rewarding.

Tbh AI getting you detected and ruining stealth would trash the gameplay experience, but it does mess with the realism ND was going for.
