Rebirth onwards put more care into item synergies so there's the extra element of playing just to see how items interact. It's hard to find another rougelike that has anywhere close to the care put into it despite how nofunallowed some design decisions are.
The other side is that beyond the "little naked crying baby game with the poop and piss" is a TON of thought out imagery that adds to it. It's not a big deal but every item and trinket builds upon it.

Lost starts with the Eternal D6 which is a D6 with two charges but with a random chance to instead delete the item instead of rerolling
Mantle is an inherent item that you unlock through Greed mode
There's a "B-side" Lost where he still retains not having a mantle and can't ever (naturally) have Mantle spawn, no ED6, but his perk is inherently removing "defensive" items from all pools

It was one of the first to really explode in popularity. Another thing is the massive amount of items that are in the game and the way that they can stack. The result is that almost every run feels unique from getting some insane combo of items. And even people who are dogshit at the game can win just by getting a lucky item combo. A shit ton of unlocks also means that there will often be a goal for the player.
The best way to summarize is that boi is really good at the stuff that makes roguelikes addictive to people. Shit's basically crack.

it's really just the sheer depth of interaction that makes the game great. it's not apparent to a new player, but when you really dig into it and understand how everything works and how you can manipulate all the elements at your disposal to make a winning run, no other roguelite compares on that front.

For me, it's the things it does that other roguelites don't. I greatly enjoy the way it feels to control the trajectory of a run in Isaac.
Secret room hunting feels like an actual knowledge check. The only other game I've seen where you can so aggressively use item pools to aim for the shit you want is a gacha. Rerolling is an actual mechanic and not an "oops, that doesn't fit in my build" mulligan.
Of course the fact that your ability to use most of the mechanics is kind of down to RNG kinda sucks, but it's not the end of the world.

Just abandon Maggie, since she'll probably drop fast and you want to save the active space for samsons's berserk instead of Maggie's heal. Thankfully Isaac won't get overkilled so bringing him back with regular healing items can work, and you can use him to keep Samson alive in turn. Otherwise use breath of life once and mantle and get good.
