I still remember how Witcher 3 had a mod where you had to shit and piss. It was hilarious, because if the bodily needs meter reached critical he would shit himself uncontrollably which debuffed him. but the way they coded the game had you auto-meditate when clearing camps? yeah. Geralt would shit himself while the 'repopulating liberated area' script hijacked control.
