I think a medieval fantasy setting would be the best. Medieval fantasy ARPGs have been done to death but for a h gems specifically, medieval fantasy would be the best because you could have fun things like bandits, orcs, ogres, trolls, minotaurs, goblins, dragons, undead and even tentacle monsters would feel quite appropriate to the setting.
Many different fetishes can be toggled on or off like BDSM, tentacles, beastiality, watersports, egg laying, etc.
There are cursed items like BDSM gear and skimpy clothes that permit putting on armor and getting rid of the curse requires going back to a town and paying the clerics a harsh fee. These cursed items could be disguised as useful armor or forced on you by enemies, mostly as a punishment for losing.

If you win a fight, you can choose to rape the enemy which will provide buffs.

You can never truly game over and the journey always carries on but there may be "soft game overs" where it is very hard to escape like being captured, having all your stats reduced.
There is also a sex system where if you rpe an enemy or do consentual sex, you can control which actions you do when.
The difficulty of the combat itself is steep and losing will be frequent when first starting out.
Also the game is fully 3D modelled and animated and the combat feels good.

Magical Girl turn based combat where there's a pain/fear mechanic. The more damage the main character suffers, the more her pain/fear raises, especially from grapple attacks. Enemies try to humiliate her, especially when fighting in public. If she suffers enough pain, she'll detransform and beg for mercy while crying and pissing herself. Another mechanic can have her detransform if she loses counscience, and you can try to retransform again but its not guaranteed, and eventually might stop working That`s all i want, its not even particularly difficult. I just love seeing pure, strong-willed magical girls being completely and utterly defeated. I have two general ideas:
An MMORPG-like world of good(pure) vs evil(corrupt). All the other 'adventurers/players' are NPCs bound by the same systems as you. An extensive character creation system so you can create you and your perfect waifu, with a character roster so whoever you create can appear as a random adventurer. Groups would be a max of 2, ideally consisting of partners or close friends aspiring to become partners, maybe some mechanical advantages to incentivize. Would have a heavy focus on social simulation mechanics, romance, imprisonment/slavery, conflict and rivalry between other players trying to fuck over other players' partners, impregnating or stealing them. Overworld would have a simulated conflict between the PvE monsters constantly pushing against the players' territory borders, able to capture villages and settlements if they're not defended enough. Game ending for whichever side completely eradicates the other. Players can ally with monsters if they become corrupted or just choose to do so.

Other idea is the entire overworld being overrun by alien/mutant insects. The last bastion of humanity having some sort of underground shelter, using what little resources they have left to clone some sort of mass producible super soldier (female). The PC super soldier is teleported up to the overworld solo Vampire Survivors style to kill as many insects as possible, attempting to clear out massive areas with each clone inevitably falling victim to the insects. Each clone gets some meta-progression points to make the next stronger, but they also ultimately become an insect broodmother, so the insects grow more numerous/stronger in the areas you lose until a subsequent clone disposes of the previous failed one(s).

Probably a bunch of details I'm missing but these are already too autistic

These are both pretty niche so it's hard to get the resources for a big game, especially an MMO. There's a mobile game that's kind of like that, but I'm not sure how well it's doing. I remember there was a huge controversy where people were purposefully sending their male characters to be captured in order to flood their enemy with gay porn. Overall, most people aren't into NTR and don't want to get attached to characters if another person is going to fuck them. It's sort of in our DNA to not want that, although some people have that fetish.

The second one is even more niche as it's a sub-genre of beastiality. Insect stuff is really niche, but a VS clone isn't too hard to do. It's the most feasible of the two.

The second one sounds pretty good and the theme could easily be substituted into other settings like a fantasy one with goblins, slimes etc if a wider appeal is needed.
Either way I'd add buildable training rooms to the base which can be stocked with enemies or equipment to provide access to H-scenes/animations. If you leave a girl in there and go on a mission when you return she'll have gained a buff or skill, potentially with some downside, as a H-themed alternate to the main meta-progression.
