They want a physical console release so they need to abide by loads of different classification boards instead of just releasing on steam and avoiding local classification altogether.

The marketing for this game was based almost entirely around how it was going to be brutal and realistic and uncompromising in its depiction of the worst that humanity has to offer. A reason was never given but based on the rating it got after the successful it appeal it was likely due to all the sex trafficking and pedo stuff. While the board is really strict about drug stuff that's only really when it's about getting benefits from using drugs not just having drugs in a piece of media.
