Meta progression as a concept is fundamentally pretty fucked up and a major flaw of the roguelite genre.
It's something that's proven without a doubt by deck builder games like Slay the Spire or Balatro, where the game makes it apparent that "Oh wait I probably would be best off with a thinner more solid deck than adding a bunch of fucking garbage to it and reducing my chances of getting the actually good cards"

the binding of isaac is fundamentally flawed. there are over 700 items that may or may not completely brick my run, and there is no way to know what they do. meaning whenever i'm rewarded it's just a dice throw whether i lose or not.

says who? These are games purposefully designed to have encyclopedic amounts of items. It's not bad game design for being purposefully obtuse and confusing. Visual novels are not bad because it's literally impossible for a single human to in one go chart out every branching path and alternative choice. These systems can be critiqued but games are neither bad nor good for being massive. It works in a game like isaac where it attempts to have a massive scope with the unique combinations you're able to make with the random pick ups as a core tenant of the gameplay experience. Knowing precisely what each item can do is really irrelevant as well as even the wikis barely scrape how the items interact with each other and player experimentation in randomly generated runs is the best method for really understanding your items and their benefits. For the basic stat upgrades the game lets you see this as well by enabling it in the hud settings

Isaac has a good story. It motivates you to play the game, to learn about his past and his fears. That on top of the satisfying gameplay loop is its recipe for success. I canโ€™t think of many other roguelikes with decent stories worth caring about. Hades was pretty good in that regard

This is why later versions of RoR2 started losing my interest. There were items which did literally nothing without a synergistic item, there were more just dogshit useless items, but at the same time the new god tier items were so incredibly broken that you either won the game when you picked up something like Pistol Shrimp on Commando, or were stuck trying in vain to make a janky bleed Sniper work.

A kinda hate the introduction of "item quality" in Repentence.
Better than the previous system of forcing only one "Good" item. But unlike a game like gungeon everything is up to chance. Sometimes I find multiple high quality items back to back and I'm not excited because I know I just won the run, there's no longer any difficulty, no longer any purpose to play the game.
I also play a lot of mods and basically none of them get item balance right. Fiend Folio will put this shit in the Boss Room https://fiendfolio.wiki.gg/wiki/Chirumiru and at the same time say this shit https://fiendfolio.wiki.gg/wiki/Golem%27s_Rock is on the same calibur as Inner Eye.
