Both. If you don't end the first level with a good amount of health, you're going to die instantly in the second. And if you pitchfork start, you obviously have nothing. Then there's issues with the flare gun not consistently setting things on fire.
Blood is extremely difficult, but in all the wrong ways. You just have to roll the dice and hope you get all the pickups you need while completely cheesing broken AI that doesn't realize you can crouch.

FPBP. Blood is truly overrated dogshit. Horrible mazelike level design, horrible gunplay, horrible difficulty optimisation, horrible enemy roster, horrible protagonist. Just a horrible game that perfectly encapulates the outdated design formula of 90s FPS games. It's easily the most overrated game on /v/ alongside Painkiller and FEAR, which is even worse for being a braindead shooter, gets shilled as a hidden gem.
Thankfully Doom Eternal saved the genre. Nothing comes close to that game in terms of difficulty, gameplay and mechanical complexity.
