To be fair, the shitshow that was XIV 1.0 was caused by outsourcing most of the development to chink developers who left it in a fundamentally broken state and produced a mountain of technical debt that Squeenix have to grapple with to this very day.

What's funny is there's always a localization fuckup in the past expansions, though it is worse now for sure.
But something I remembered randomly is when 3.5 came out with a massive QOL. Previously, those FATEs that start by talking to an NPC only showed up on the map if you were nearby the spot. But this QOL made the icon (the small exclamation mark) visible from anywhere on the map.
It was a huge change, and the English Patch Notes completely didn't have this line translated.
