I think it's more about confronting the man James himself rather than just the Mary murder. His alcoholism, infidelity, and self image are constantly forced upon him in physical form to make or break him. It's what the kids these days would call "kino". Japan rarely ever can pull off such mature storytelling without their classic Japanese humor moments that completely ruin the vibes. Silent Hill 2 is throughly oppressive and thoughtful about it's narrative. It's vidya writing at some of it's best.

The game itself also suggests this with James leaving the room with her body in In Water (this is the suggestion that the BoLM gives, that he's going somewhere with it, not very difficult to figure out). This is further "confirmed" (if you're willing to accept it as a source) in the 2024 Remake where Mary's body is literally visible (under some sheets) in the backseat of the car and is very not subtly pointed out in some endings.

He's walking through fog, he's in a dream. He starts the game at the overlook holding a letter and in all of the non suicide endings he ends the game there and is handed a letter by maria or laura. All the characters in the game know about him and mary, their weird cryptic dialogue isn't an accident, they're all forms of 'the crimson one', they all have the color red on them, the save points are all red. When james meets laura he's standing in front of a wall with a picture of a bear and a cat in the same positions that he and laura are in, surrounded by moths. You see this same motif over and over again, laura playing with bears, leaving them behind or drawing bears and cats everywhere. You find one of these bears in the apartment with the mirror room, angela leaves the mirror room and James is left to stare at himself. The mirror room has a picture of two blond children with the backstory that their father abused them, angela has black hair. The bloodied man sitting in front of the TV is most likely james' father. James fights 'angela's monster' over an over again, angela isn't real, it's his monster. Eddie and angela are always in his path waiting for him, the whole thing is a show for James. James meets Eddie in the prison which is not possible because the prison isn't real. All the corpses in the game are dressed in james' clothes. And so on and so on, if you don't ignore all the small details that don't add up in the game the obvious conclusion is that james is dead and is wandering through purgatory while being tested by the 'crimson one' until he relieves himself of his attachments to his past life.
