Idk, like most things in MGSV, it's better on a replay after the twist.
Especially so the very nichely known detail that he puts quotation marks around "big boss" after and only after you complete mission 51, heavily implying he knew you weren't big boss, and didn't even care. He was empathizing with you.
Almost every major cutscene in MGSV takes on a double meaning after the twist, and you are intentionally supposed to feel empty after skullface, I think.
