Madoka by far. With the exception of Precure, up until the release of the Cardcaptor and Sailor Moon remakes, every mahou shoujo property tried to cash in on the deconstructionist aspect of Madoka while ignoring everything else about it.

None of those really. Even Madoka would've been alright in a vacuum.
The true downfall didn't really start until mahou shoujo site.

If you want to point to a series as the "catalyst" though, that trophy belongs to Nanoha, not Madoka.
Nanoha was the one that showed everyone mahou shoujo didn't have to be mahou shoujo to get people to slurp it up. That it was in fact fine to turn it into a fucking shonen series or whatever the hell you wanted.
