What exactly are you supposed to do in this game? It hints at the idea there's more to it than the "main" quests you can take, but I can't seem to figure it out. Is it just going around killing things? Seems kind of deceiving if that's the case.

The only way to find some minor enjoyment out of games like that is to contract autism+ and keep that perk for at least 10 years.

I think this game has an ending so the question is dumb

Just play Elin bro. You're not gonna get a straighter answer out of this than that. Game's 100x better than this and is a genuine sandbox genre.

talk to "people" in towns and just explore random areas like caves.

There's nothing sandboxy about Elin, it's just Noa's misguided take on Stardew Valley with a mediocre roguelike minigame to break the monotony of your fantasy tax reviews.

>Find the overland locations or tribe bosses, kill them for gear
>Do the main or side dungeons
>Build your character, hunt for cybernetic tokens, implants, etc
>Do the side quests, like the deer bitch's quest in the desert city
>Spread Klank
>Kill shit deep underground

The gameplay loop is a bit tiring after so many hours, wish it had more side quests and few other world maps to explore, more skills or builds to try out. It's got great bones and some decent mods make it even better. It loses its novelty pretty quickly after you get the core mechanics down, like 30 or so hours in with successful characters. Still the best roguelike to play occasionally, hopefully they make an expansion or something.

There's the slynth sidequest, killing or befriending the Nephilim, the optional stuff in Chavvah that ties to Ptoh, Mamon's quest and a couple of other things, with mods you open up a hell of a lot of possibilities too.
But yeah, after some 500 hours or so vanilla Qud does get a bit monotonous, I'm still annoyed they took out the Trembling Dunes entirely, but supposedly we got a few expansions to look forward to and they already said they wanted to bring them back, so there's that.
