How do you feel about tattoos in video games? When it's an optional thing you can get for your character like in GTA?
What about when it's a lore thing?

In pic related for example people put these tats on their body so they can have magic powers. But aside from looks and a bit of extra lore nothing is done with these tats. There are no mechanics involved with them; it's purely a surface thing with no substance.

Depends on the game. Fable had some minor mechanical elements with tattoos iirc. I could see a Yakuza game where tattoos act as a sort of journal.
Overall, the main issue is that if there are mechanics involved, then that leans towards the tattoos being (almost) mandatory. That means that the tattoos need to fit the theme and characters strongly, and if it's common among all characters, then there also needs to be enough variety among them to not look samey (Yakuza is again a good example where that works). And since it's balancing visual elements with mechanical elements, it suffers from similar effects as other equipment, which can lead to transmog being welcome.
