Dishonored was infamous for this but you can look at just about every title with an honor / karma mechanic over the last 15 years and it's all the same shit. The only game I can think of with a honor mechanic that isn't like this is red dead redemption 1/2

Becuase it's the intended oath bybthe devs so of course they will have more finished and curated content. Where as the opposite will have less or locked out content.

Two prominent examples are Fallout New Vegas and Baldurs Gate 3.

It could be done by people you know, people you don't know, people you only know about(think politicans, lawyers), OR it could refer to natural disasters. It could be self inflicted. It could even refer to accidental suffering, say your dad accidentally fell off a roof and died, which brought years long poverty upon your family. How do you get your revenge in that case?
