You have to complete a few of the repeat missions. They're there as stand-ins for the Kingdom of the Fly missions that were cut earlier on.

That's not quite it, you just have to get her above a certain level.
The butterfly thing was that originally, you could put the emblem on to prevent her from leaving, but then you could never finish the game because she's supposed to leave.

Originally on release, the intent was that she leaves and then she's gone forever, and people bitched so hard about it that much later on when Konami took control they patched it so that you can use it to get her back after she leaves in the story, because people were just hacking her back in anyways.
An upgraded D-Walker is supposed to overlap with her somewhat but people thought it was lame.

The current version of TPP is somewhat different from how it was on release.

This game was my personality in 2014-2015. I remember smoking a lot and listening to Sins of the Father and Quiet's theme on repeat. I didn't even play any of the Metal Gear games, except MGR and Ground Zeroes. Venom Snake's autism just spoke to me. It was Drive of vidya.

You know, perhaps there could've been an interesting story narrative on how a doctor, while important, is essentially a bobody who helps others. A visible phantom who eases pain, trying to heal others from their own phantom pain, to alleviate his own pains and fears that haunt him. To being someone who then ends up forcibly becoming an icon for a war mongering monster, a demon who projected his own sorrows and frustrations onto a new vessel, as if to escape it, to use him as a basis for a haven he can escape to. A man made Valhalla. Hands of kindness that turn into gauntlets of death, all just to try to forget the suffering in their lives, the ones they couldn't escape, and the ones they create, either through maliciousness or ignorance.
There could've been something genuinely beautiful. Kojima could've made the first video game that would undoubtedly, unabashedly be "truly artistic".
Instead we got "2deep4u" surface level narratives and an unfinished piece of shit. All because he wanted to blow most of the budget on B-list euro actors and some random american actor.
