I would get scared when I'd collect all the hearts and their eyes opened.
For some reason the spiderbot enemies in Deus Ex made me uncomfortable. I think it's part their metallic crawling sounds and part how fast they were moving. Whenever I heard them crawling around, I'd often just go out of my way to find a different route to avoid them.

Not a scary game or enemy in anyway but I was looting fishers shack in new vegas and the whole time a lakelurk was just standing in front of the door and it made me jump out of my fucking seat because I only noticed it when I was leaving

they become retarded once you get sentinel armor though, they run away from you instead of attacking unless you stand right next to them, so they know they can run away
scary enemy though because of this they tend to just appear out of nowhere or you can find them just standing two tiles behind you, watching you

Running into this freak during a random encounter
This, from Zelda 2. I watched my dad play and leaping down a pit scared the SHIT out of me; it was a combination of the sudden loud sound and unexpected visual with a wounded-sprite Link, I thought it was some death scene. Weirdly got over it in like a month tops though because I wanted to play so much.
It is literally the easiest and shortest level in the game but as a dumb kid I refused to do this mission and always skipped it
as someone who has a phobia for dark or deep water...walking down to some huge, dark, murky, abandoned lake to fight a fucking giant water snake was NOT the kind of vibe i wanted.

I'm completely serious, a goddamn Minecraft spider terrified me. Nothing else in that game phases me, just the damn spider. I already had a problem with spiders before playing it, but its those damn sounds they make that make it so much worse.

I canโt find a pic but there was this really creepy thing in SOTN when you entered this room and I think sat down. And it would either be a good version and sit with you or it was a bad version that killed you with spikes. That thing always creeped me out as an adult and it has creepy music playing too
