Honestly, I feel like this was something they definitely should have improved on. There should have been more unique and cool rewards on the map.
But the exploration is so fun that I still cleaned out the whole maps anyways. A "doing it for the sake of doing it" sort of thing.

We'll probably never get a sequel that adds more classes and more spells and abilities, but I was satisfied enough with what we got, I played the Pillars games as a Monk so I didn't mind switching up to Mage this time around.

The unique loot is predetermined, although I'd say only like 20% of the unique loot feels like it has anything to offer beyond its flavor text. Just got a dumb ring that was "Enemies have 10% chance to miss you with ranged attacks". Game has worthless items like that.

For the non unique loot, it's not randomized but it's scaled, at least as of the original release, they may have changed how that works with patches.
