He was abandoned as a child and raised in the sewers by omens. This led to him identifying with omens and hating the golden order and the surface dwellers for their mistreatment of omens. He somehow developed a curse that will allow him to force people to reincarnate as omens after they die. Normally bodies are ritually buried by Erdtree roots and consumed by the trees which allows them to reincarnate healthily, but he shoves something up their ass which prevents this. Reincarnating as an omen is a fate worse than death. His mending rune afflicts everyone with this curse. Sort of a "if everyone is cursed, no one is". Omens live in constant pain from the horns that grow from their skin so it is very undesirable for reasons other than just social and psychological.
