Healing is thoroughly explained in-game if you level a healer. Healing does not magically regrow limbs and close wounds, it only accelerates natural healing processes, of which there is a limit. Can you naturally put together your shattered shins? No? Then neither can your local white mage or scholar.
There are several cutscenes in which healing is applied to a character to stabilize them, and are then promptly told to fuck off and rest because despite the fact you're no longer bleeding out, you are stilled fucked.

Arenville got his fuckin lower spine shattered. You don't 'heal' that in the story's setting.

Nothing. In fact it's the optimal way to play because it's so ludicrously overpowered. The best way I saw someone put it is to strip away the wheelchair itself and make it a sword, for example.
So you show up to a game and speak with the DM and demand he let you start with a homebrew sword. This sword is indestructible except for 3 consecutive critical hits. This sword lets you hover at will, raises your move speed to 30 if it's less than that, does not need to be wielded in your hands, you gain advantage on saving throws to be knocked prone, gives you a bunch of free skill points, and can be upgraded to give it better attacks, ranged attacks, free spell casts, allow you to walk on water, get automatic cover from ranged attacks, and armor bonuses. it does not need to be attuned and carries over into any wild shape or shapeshifting effect on you.

I'd get about 3 items onto that list before the GM would tell me to get the fuck out of his game and never come back. It's obscenely overpowered.
