Healers have extremely long ults so rather than address the ults massively stalling everything just nerf how long it takes to get the ult so people still stall the game it just takes a bit longer. This doesn't stop Loki from just stealing said ult and doubling the time the opposing team can't do anything.
Jeff and Thing were both released with what felt like an unfinished kit and took until 2.5 and 3.0 respectively to finally get kits that make sense.
Bucky has been consistently ridiculous since launch but he's been either untouched or outright buffed at times.

Spidey can singlehandedly stop entire teams from winning if the player understands how to play him yet never really gets nerfed leading to instabans constantly on comp and on QP you just pray the Lord Spidey you see if having an off day or you're not doing anything.
Venom and Thor have consistently been fairly lackluster tanks yet the former hasn't really had any changes and the latter keeps getting weird reworks that typically make him worse.
Storm and Namor released more or less unplayable and took until 1.0 and whenever Namor got the Hulk teamup to become decent. Now Namor is solid even without the team up due to the speed up on the trident but it took ages to get to this point.
2.0 was dominated by the triple healer meta leading to dives so they spent 2.5 nerfing dives hard.
2.5 was dominated by multiple fliers thanks to Ultron leading to extremely cancerous games where 75% of the team couldn't do anything to kill these bugs in the sky while they chipped away at you from miles away.
