Biggest issue is creating actual game content in meaningful way.

The main reason why the video got so viral is because it showed just enough to spark everyones imagination. How big the world is, how much you can discover, how the combat works, how much non combat stuff there is to do, all of those things were filled by peoples imagination. If you try to turn it into an actual game all of that magic will quickly be gone and it'll have to measure up against other games in the genre and it'll most definitely fail in every regard because open world first person rpgs are some of the most difficult games you can make and the dev team is a bunch of idea guys and volunteers from twitter.
