Rape is basically impossible to justify under anything less than an extreme circumstance. It's basically comically evil to rape, unlike murder, which is easily understandable as a motive.
Given that unjustified murder is well beyond enough to establish a villain, there's no need to introduce rape. The only reasons narratively for it occur in a story, is to portray the villain as specifically sexually evil, which serves little purpose in 99% of video game scenarios, or to give a non-villainous character a sexually traumatic backstory, which also usually serves little purpose.

Depending on the pulp story. By and large you're right. I helped a guy compile a list of some of the more controversial ones and track them down for his collection a few years back. Some of them were banned, others just had bad reputations. People are weird they have a sliding scale. An adult woman is more acceptable than a fertile teen is more acceptable than a fertile tween is more acceptable than an infertile preteen is more acceptable than toddler is more acceptable than an infant. Once you get into the realm of fiction people's priorities usual change unless they're morally grandstanding or trying to use it as a sword and shield to get something done. Because as nasty as the subject matter is in fiction it's still not real so usually the "they wrote or animated this horrible thing!" isnt enough which is why you see social groups and politicians connect it to real stuff insisting the fiction will cause people to do the real stuff.

Which doesn't pan out, Japan for multiple decades conducted studies that have pretty much proved negative behaviors having a fictional outlet drastically reduces the real life instances of those negative behaviors. One or two or three examples wouldn't work, but the Japan have shown consistent results on this one so I'm willing to believe they're right.
