It's similar to a Patronus. You have to power a Patronus with a happy memory and if you don't get in the right state of mind the spell fails or is weakened.
To Abra Kadabra you have to power it with murderous intent.
To Crucio you have to power it with a desire to cause pain.
Naturally, evil psychopaths are better at casting these spells.
Harry was able to Crucio Bellatrix for a short period using "Righteous Anger", but it wasn't very effective. To use it properly you have to really enjoy it.

There were a few occasions in the books where Harry tries to use a Patronus, but it takes him a few tries because his life is so miserable. If his opponent was a wizard firing spells at him instead of a dumb dementor he would have been killed by those screw ups.
