Matchmaking ultimately caused it. If you routinely toss unrelated retards together, then eventually everyone views everyone else as hostile and starts coming up with ways to punish non-compliance. Providing rules and guidelines funnels those punishments and made-up rules into something approaching an almost logical (but still fanatical, everyone still hates everyone) framework, rather than an unbounded cesspool where everyone has somehow slept with your mother
Ultimately matchmaking is still completely untenable and you need a dedicated server browser, but it will at least tide the game over until the company has extracted profit.
