My best explanations:
It's theorized in the conspiracy route that an anti-human element may have intentionally sabotaged Eitp's memories in order to jeopardize the mission. Also, Shouma acquired his self esteem issues just because some employees made fun of him while he was in his pod, so it's not an exact science
The Vengeance ghost is in Takumi's head, but he was CREATING that ghost with his hemoanima. It's like if someone made a puppet and started talking through it.
The game never explains the PLM lab. Either sequel bait or a plot device they couldn't explain.

A lot of this is explained by coming to terms with Kodaka's routes being the "canon" routes and Uchikoshi and cos being non-cannon fanfiction routes. In Kodaka routesThere is no time loop, Hiruko doesn't have a going to other worlds machine, there are no Gie. If something isn't in Kodaka's routes then it doesn't exist in them. A lot of the confusion in other routes stems from the other authors trying to take in consideration all of the other routes while writing their own, which gives the player the impression that all of the routes are connected in some way and to expect answers to lingering questions or why certain plot elements are completely absent in many of them.

tl;dr half of the game is Uchikoshi fanfiction that feels like it's completely unfinished and the rest of it is the other lesser known authors writing filler episodes for a 1000 episode shonen anime.

There's something like 7 families per kid up for adoption currently in the US. Numbers aren't going to be much different in other first world countries.
