Two things
>No "uwu soft femboy" shit, Lisa is a game about sweaty, masculine men wrestling, chugging beer, and violently beating the shit out of each other
>No nonbinary shit, men are men and women are women. No trannies, just dudes crossdressing and dressing in drag to cope with the lack of women

Due to its premise its entirely populated by ugly men, instead of ambigiously gendered teenagers or cute kids
The honey to speculate about gender and form fanfics around it just isn't there
