Basically the setting takes place in cycles. It seems possible there could be other types of cycles, but as of yet the only two depicted cycles are the duality of killing/non-killing. In prehistoric times the cycle had been in the killing phase, until some turning point after which the cycle entered the non-killing phase. This may have resulted in the advent of civilization, but in any case the world has been in the non-killing phase of the cycle up to present day. Go anywhere in the world, killing and murder were not the guiding principles/force. This cycle was complicated by an "entity" or presence referred to as The Evil Metaphor for History, and so while the cycle was in the non-killing phase, History is still a brutal and bloody thing rife with killing and murder. Notably though it is not the guiding principle/force of the world, so in the same way that in the killing phase of the cycle there can be instances of non-killing, then in the non-killing phase of the cycle there can be instances of killing. It's not what dominates the phase of the cycle, it's just something that bleeds through from one another. Perhaps that the cycles are dominant, but not 100% strict or definite? In any case it's not important.
So during the current phase of the cycle, non-killing, History and particular agents of it have been trying to flip the phase of the cycle back to killing. These efforts have culminated in the status of XX City as an oasis of killing and murder in a non-killing world, and perhaps most importantly the incarnation of a (meta)physical avatar for History in the form of: The Killer. Notably this is not the first time The Killer has been incarnated through the course of history, but the current incarnation was intentionally brought about in the most ideal place for it, the haven of death that XX City has become, in order to bring about a change in phase of the cycle.
