This happened originally with morrowind modding, overall TR managed to figure it out by not having as many central figures. Saw the same issue with Fallout the Frontier.

Why does the unofficial patch exist? According to its purpose it fixes minor bugs did Bethesda just give up patching the game after release? Isn't this their job?

In the 90s and early 2000s they were often more like Libertarians of the time, so more intelligent,knowledgeable, and wise than most commenters. There was a little gladhanding backrub toxic positivity shit but those engaged in it were often genuinely respectable forum members themselves. Many mods and admins prided themselves on their objectivity and true objectivity was something to strive for.
Not some golden age but far better and different than what you have now which is either barebones lets not have the fbi visit twice in just one month moderation or full on admin abuse we are all used to.

Fallout the Frontier was made by a team of modders, the world was handled by the lead, but everything to do with the NCR was handled by a guy who didn't talk to anyone else and huffed his own farts all day. The kind of guy who credits himself as "the student of Kojima" and the entire NCR campaign was basically a family guy episode played straight because everything is a reference to a war movie or a game. You get the Wolfeinstein reboot "choose which friend dies" scene, and after a couple rooms, you do the dead space 1 meteor defense mission. Every single scene in the game can be pointed to be a reference or 1 to 1 recreation of something else and the NCR's end goal is to restore a SHIELD helicarrier and then nuke Shady Sands. It's all played straight and is hiliariously bad. Who could be the Legion spy? Surely not the second in command who wears sunglasses, doesn't talk much, and has a Latin name. Oh also they all jerk off how cool and awesome the courier is, but the legion armor has a stupid high DT so when you play as the Original Characters instead to save the Courier, they all have DT ignoring weapons so they feel stronger and cooler and make you look like a bitch after saying how amazing you are. The Legion campaign wasn't much better, the legate, despite there only being 1, in charge of the campaign is progressive and is phasing out slavery and bringing about gender equality, socialized healthcare, and mass education... to the Legion. They have advanced weapons... which is just Helghast armor. All of the Legion's gear is asset rips of someone elses mods importing stuff from other games. The Brotherhood campaign was played by basically no one, because they're Militant Mormons and they got OC do not steal weapons including a laser rifle with an Alien's motion tracker on it. Everything is justified by "oh yeah we had a bunch of cutting edge super advanced prewar weapon caches here in Portland fucking Oregon

Now, the vehicle system was made by one guy, who was already making the mod but they paid him to lock it to theirs. A bunch of other modders came in and added stupid shit but the most famous is the snake people in the sewers who are all addicted to drugs and have a fetish for human men. It's a unity asset store asset with mentat containers stuck on it and they're treated as a major player in the region, despite living in the sewers under the NCR base, and doing nothing but trying to fuck humans and doing drugs. They're never mentioned by the Brotherhood or the Legion but that's apparently how the NCR gets funding. Then there's the Brotherhood Companion, a girl who has an 18th birthday card in her inventory because she acts like she's 12. All of her content is fetish bait. She talks about her smelly feat, and her quest is you doing skill checks to turn her into your little slave girl. They defended how creepy this was by saying "it's like, a joke, because you normally do skill checks without thinking so what if the skill checks were bad?" but that is flimsy at best. There's also the "Oh that was supposed to be locked to wild wasteland" things like where you run into a death claw and all your speech options are how you convince it to have sex with you. There's a quest where you hunt a Yeti because those are real. A pirate cove full of yar har type pirates, a tribal clan that apparently did blood transfusions and then all died of aids but the last guy who wants to do it to you because bug catchers. It's all degenerate and creepy and gross

The actual project lead made a submod, and if you don't install that submod, a massive chunk of the map is blocked off, because the submod is a quest about being turned into a slave and is actually really well done. It was designed as a vertical slice of what he wanted the mod to be, with various skill checks and alternate routes to finish quests. Like if you find something, the quest can go five ways depending on who you tell, when you tell them, how many you tell, or if you don't tell anyone at all etc. But it's limited by the engine because it's still the fallout 3 engine, it isn't built to have that many branches

I think it just comes down to modders doing it “for free” to be honest. But they subconsciously think they deserve something for putting in so much time for their “work”. All humans expect compensation or at least appreciation for the time invested into something. But modding doesn’t bring monetary compensation in the same way, say art commissions do for Twitter/X artists for example. So modders want recognition and respect from the community instead as a form of compensation in a backwards way.

Honestly who the fuck would want to be a leader of a modding community outside of a weirdo who wants to have power over other people? People who just like to make mods just make fucking mods, they don't try to gain authority over modmaking or any other shit. A lot of players view themselves as gods, that they're doing a modder a favor by installing their work, and that they should just swallow all the shit that players feel like flinging their way whenever.
